With Positive Airway Pressure, more commonly called CPAP or PAP Therapy, we have efficacy but chase compliance. Said another way: PAP Therapy works for those patients that are able to tolerate it. Although we have made "great strides" in this technology, still studies demonstrate 50% quite their prescribed therapy after the first year and that number reaches 83% at the five year mark.
Although Oral Appliance Therapy provides a much more comfortable option it is said to not be as effective in lowering patient's indexed measurement of the disease while at the same time improving subjective symtoms and quality of life indices. Still it must be said that with PAP Therapy "we have efficacy but chase compliance" while with Oral Appliance Therapy "we have compliance but chase efficacy". This makes success less related to which form of therapy is used but rather more related to the skill in knowing in advance how to select the device that "best fits" each individual patient's circumstances.
